The Care Process
- Give us a call and feel free to ask as many questions as you have. Dont forget to ask us about our several free membership benefits.
- We will then set up FREE no obligation In-Home Assessment appointment at your convenience and our expert Home Care service coordinator will come to your home. This evaluation can take 1-2 hours. We encourage family to be present during this assessment. To better serve your needs, our Home Care service coordinator will learn all about you: your medical condition and history, medication schedule, daily routine, emergency plan, family involvement, and your preferences and expectations.Once we know what your home care needs are, we will explain our membership packages and give you a quote. We will need a signed contract to start.
- A care plan is then built according to your medical condition and disability needs along with your preferences and expectation and we begin to match your needs with our specialty in home care providers. It can take up to 48 hours to find a match and get you on their schedule.
- We will reevaluate the progress and services our providers have given and ask for your feedback at the end of each month and generate a report for you and your doctor, if you request.
